Is the use of local anesthesia containing Adrenaline during minor oral surgery cause hemodynamic changes in medically complex patients

Objectives: This study analyzes hemodynamic changes in patients undergoing oral surgery procedures following local anesthesia with Adrenaline
Methods: This randomized clinical trial was performed at the Department of oral and maxillofacial surgery in the school of dental medicine Tel Aviv University and in Assaf Harofe Medical Center. The study group consisted of medically complex patients (cardiovascular conditions) ASA II and ASA III . The control group consisted of healthy patients ASA I. The Patients underwent minor oral surgery procedures like: tooth extractions of all types, oral biopsies, dental implantation and bone grafting. They were monitored for hemodynamic changes: blood pressure, heart rate and oxygen saturation before and during oral surgery following local anesthesia containing a Adrenaline ( 2 carpules of Lidocaine 2% with epinephrine 1:100,000). The fluctuations of theses parameters were compared to the initial values prior to the local anesthesia and before and during the procedures. The patients' status (blushing, pallor, sweating) and level of pain and anxiety were recorded.
Results: The study group consisted of 45 medically complex patients . The control group consisted of 56 healthy patients. There were no differences in the systolic and diastolic blood pressure in the heart rate and in the level of oxygen saturations in the study group before, following local anesthesia injection and during all stages of procedures. All the patients were stable hemodynamically. There were no significant differences between the study group and the control group during all the stages of operation.
No statistically significant differences were shown in blood pressure, heart rate and pulse oxymetry values before, during and after local anesthesia injection between the study and the control groups.
No clinical signs as blushing, pallor and sweating were found in both groups.
Conclusions: Lidocaine with epinephrine (1:100,000) provided effective local anesthesia. This treatment did not cause an increase in heart rate or blood pressure and did not change the blood oxygenation level in medically complex patients and in healthy patients thus indicating that the use of lidocadren 2% (2 carpules) is safe in medically complex patients.
