Dimensional changes of the alveolar ridge in the posterior maxilla and sinus pneumatization following teeth extraction
Objectives: To examine radiographically, pre and post extraction dimensions of the alveolar ridge and maxillary sinus in the posterior maxilla using a novel super-positioning method to standardize the panoramic radiographs.
Methods: twenty two pairs of panoramic radiographs were analyzed retrospectively in order to measure changes in the maxillary sinus and alveolar ridge dimensions, before extractions and at least 6 months post operatively. The two radiographs – before and following extraction were superimposed and matched using a fixed reference unit. After matching, the following measurements were performed: The distance from the bone crest in the middle of the extraction site to the sinus floor and to the sinus roof, the distance from the sinus floor in the middle of the extraction site to the sinus roof and the maxillary sinus sagittal circumference.
Results: The mean change in the distance from sinus floor to sinus roof pre and post operatively was 1.02 ± 1.5 (4.59% ±6.8%), p=0.001. The mean change in bone crest to sinus roof was 0.91 ± 2.7 (2.88% ±8.0%), p=0.094. Likewise, the mean change in the distance bone crest to the sinus floor was 1.23 ± 1.84 (11.71% ±22.3%), p=0.001. The mean pre operatively sinus sagittal circumference was 993.94 ± 295.7 mm while post operatively the sagittal circumference grew to 1096.57±312.5 mm, p<0.0001.
Conclusions: A moderate increase in the maxillary sinus dimensions together with concomitant crestal resorption may be anticipated after the extraction of maxillary posterior teeth, all leading to an overall decrease in the alveolar bone available for implant placement.
