Guided bone regeneration of human infected bone defects

Objectives: The aim of the present study was to evaluate the new bone formation potential, of combined bovine bone scaffold with soluble regulators acquired from blood products and Metronidazole, in human infected bone defects.

Methods: Study population was divided into two groups: Infected sites (i.e after tooth extraction, infected implant removal) and control non-infected sites (donor sites 6 months post augmentation procedure). All bone defects were filled with bovine bone scaffold mixed with Metronidazole. Either Platelet Rich Plasma (PRP) or Bone Marrow Aspirate Concentrate (BMAC) were used as soluble bone growth factors and stem cells. Bone biopsies were taken 6 month post augmentation (at time of implant placement or second tier augmentation procedure) using a surgical trephine burr (3 mm diameter).
Biopsy specimens were stored immediately in 10% buffered Formaldehyde. Histomorphometry of non-decalcified samples, using light microscopy after Acid-Fuchsin and Toluidine-blue staining, was performed measuring the newly formed bone, residual grafting material and marrow spaces.

Results: Most of the biomaterial particles were surrounded by newly formed bone with large osteocyte lacunae. In the same areas the presence of Osteobalsts deposing osteoid matrix in direct contact with the biomaterial and bridges of newly formed bone between the particles could be seen.

Conclusions: New bone formation in infected sites following augmentation with bovine bone saturated in soluble blood products, did not differ from non-infected control sites.
