Treatment of Langerhans' cell histiocytosis with Maxillofacial involvement
Objectives: Langerhans' cell histiocytosis (LCH) was formerly known as histiocytosis X occurring at prevalence of 1-2 per 100,000. It is a group of conditions characterized by single or multiple osteolytic bone lesions with or without visceral involvement demonstrating infiltration with histiocytes. Involvement of the maxillofacial bones in LCH may cause complaints of facial swelling, hearing loss, gingival bleeding, pain in jaw and loss of teeth. Posterior bone regions of the jaw are affected more often than anterior regions. The purpose of the current report is to describe a case in our department of a LCH which involves the maxilla and mandible, clinical treatment and 8-year follow-up.
Methods: An 11-year-old girl was referred to our department at 2007. She was diagnosed with history of LCH since age of 3 years, with multifocal involvement and systemic disease treated with chemotherapy. At the time of admitting her to our department she was free of the disease. Clinical and radiographic examinations have revealed severe maxillary hypoplasia, madibilar body hypoplasia, and loss of multiple teeth in both jaws. The treatment plan included at the first stage orthodontic treatment including rapid palatal expansion, distraction osteogensis of the maxilla. At age of 17 she underwent an autogenous bone graft for both jaws and dental implant insertion in the mandible utilizing the lateralization of the inferior alveolar nerve. Results: After orthodontic and surgical treatment, the skeletal position of the maxilla was corrected with improvement of the face profile. The missing teeth in the lower jaw was restored by successful bone grafting and dental implants.
Conclusions: In this case report we demonstrated an 11-year-old girl diagnosed with LCH treated and followed up in our department for 8 years. The deformity correction of the maxilla and mandible and the restoration of her missing teeth has surly improved her quality of life. LCH patients with maxillofacial involvement require a clinical treatment plan with a multidisciplinary management including orthodontics, maxillofacial surgeon as well as prosthodontics
