Circumferential Supra- crestal Fiberotomy Orthodontics enables short orthodontic treatment

Objectives: Rapid treatment and long term stability are currently considered as essential requirements of orthodontic treatment. CSFO is designed to substitute Periodontally Accelerated Osteogenic Orthodontics (PAOO), in an attempt to achieve these objectives. Secondary possible objectives are reductions in the need for tooth extractions and for orthognatic surgery in borderline cases. PAOO is a combination of orthodontic treatment with extensive periodontal surgery, consisting of corticotomy and bone augmentation. However, some patients are reluctant to undergo this treatment modality due to increased morbidity and higher cost. CSFO incorporates a minimal surgical intervention which has been used for almost 50 years, to prevent rotational relapse of orthodontic treatment. The surgical procedure is performed by inserting a blade into the periodontal crevice for the purpose of detaching the epithelial-attachment and the supracrestal and transeptal fibers around each tooth. Recently CSFO has been proposed as an alternative to PAOO due to its acceleration of alveolar bone turnover which enables rapid tooth movement. CSFO was found to both accelerated tooth movement and reduced post orthodontic relapse in a rat teeth model.
Objective is to demonstrate that Circumferential Supra- crestal Fiberotomy Orthodontics (CSFO) can be used to shorten the duration of orthodontic treatment.
Methods: To treat orthodontic patients in various therapeutic approaches - non extraction philosophy, lower incisor extraction and four premolar teeth extractions- utilizing CSFO
Results: Three patients were treated using CSFO. Treatment principles for achieving rapid tooth movement, shortening orthodontic treatment duration and long term stability will be presented for CSFO and PAOO.
Conclusions: CSFO may be considered as an alternative modality to PAOO for shortening orthodontic treatment duration.
