The Management Of Complications Following Dental Implants Migration Into The Maxillary Sinus.
Objectives: to investigate the complications following dental implantation and migration into maxillary sinus and their management in order to elucidate the reasons for the complications and enhance their prevention. Methods: a retrospective study of case series of patients treated by specialist of oral and maxillofacial surgery due to dental implant migration into maxillary sinus between the years 2000-2012. demographic and clinical data were retrieved from patients files. Results: 55 patients were included in the research 30 males 25 females average age was 58 years(36-84). Oroantral communication was found in 46 cases more in cases without prior bone augmentation, in older patients (mean age 60) and medically compromised patients (ASA>1). The patients also presented sign of maxillary sinusitis like headache, nasal obstruction, nasal drip. Headache was found mainly in women (P=0.033). The migrated implant was removed from the sinus mostly by Caldwell luck operation. The oroantral communications was closed by local flaps- buccal and palatal flaps and also buccal pad fat flaps and combinations. in most of the cases (32 out of 46) the oroantral communication was closed by a single operation in 14 cases the communications were closed by more than one operation. in all the cases the oroantral communication was finally resolved. Conclusions: Oroantral communication and maxillary sinusitis are common symptoms following dental implant migration into the maxillary sinus. The risks factors for this complication are: dental implantation in the posterior maxilla without sinus augmentation older and medically compromised patients. Closure of the communication is performed usually by local flaps, buccal and palatal flaps and by buccal pad fat flaps.
