Platelet Rich Plasma and Bone Graft in Aggressive Periodontitis Therapy

Objective: A combination of platelet-rich plasma (PRP) and bovine derived xenograft (BDX) has been shown to be effective for the regenerative treatment of intrabony periodontal defects in chronic periodontitis patients. The aim of the present study was to assess the clinical and radiographical outcome of this combination in the treatment of intrabony defects in generalized aggressive periodontitis (GAP) patients. Methods: Twenty GAP patients who received non-surgical periodontal therapy and had radiographical interproximal defects with an associated probing depth (PD) of > 6 mm and an intrabony component of > 3 mm, were included. A total of 100 defects met the criteria for inclusion. Each subject exhibited at least 4 intrabony defects. Prior to surgical treatment and at 12 months, plaque and sulcus bleeding indices, PD, marginal soft tissue and relative attachment levels, probing bone and radiographic bone levels were measured. Results: Favorable clinical outcomes in soft and hard tissue measurements were achieved when compared to baseline (p<0.001). Considering the deepest site of the defects, 4.51±0.43 mm of PD reduction, 3.84±0.14 mm of attachment gain, 0.61±0.03 mm of recession, were obtained. The mean clinical and radiographic bone gains were 3.34±0.01 mm and 2.96±0.28 mm, respectively. Conclusion: According to the results of this study, this combined technique seems to be an effective modality of regenerative treatment for intrabony defects in GAP patients. Further randomized controlled studies are necessary to elucidate the role played by each component of the combined therapy in achieving these results.
