Fluoride and Chlorhexidine mouthwashes and inflammatory molecules

Objectives: Peri-implantitis is an inflammation of tissues around implants characterized by bone loss mediated by pro inflammatory molecules such as IL-1&beta, PGE2, VEGF. The aim of this study was to evaluate the influence of Amine Fluoride/Stannous Fluoride (AmF-SnF2) versus Chlorexidine(CHX) combined with Am-SnF2 on IL-1&beta, PGE2 and VEGF secretion by cells of crevicular peri-implant fluid.Methods: 30 patients rehabilitated with dental implants were enrolled for this study. GCF samples were collected at baseline (one month after fixture uncovering and placement of healing abutment), at day seven and fourteen. Patients were randomly assigned to sample or control group.The sample group used AmF-SnF2 rinsing for 1 minute twice day for two weeks, the control group used CHX 0.12% in t first week and AmF-SnF2 in second week following the previous regimen. Amount and concentration of each cytokine in the groups were calculated as subject mean ± standard deviation. Variation of pro- inflammatory molecules in function of time was calculated with paired t-test. Total amount and concentration of each cytokine was compared between study and control groups using unpaired t test. The level of significance was set at p≤0,05 for all tests.Results: IL-1&beta, VEGF and PGE2 levels were significantly lower in test than in control group. Comparing first and second week of treatment, it was evident a greater decrease of IL-1&beta and VEGF in sample group during second week. There was a lower decrease of IL-1&beta and VEGF during the entire treatment in control group. Differences of PGE2 levels after 7 days in both the groups were not significant while there was a significant difference during the second week.Conclusions: The data suggest that the use of AmF-SnF2 could decrease the production of Il-1&beta,PGE2 and VEGF by inflammatory cells.AmF-SnF2 could be an alternative to CHX mouth rinses in plaque control of patients with implants.
