Objectives: Teeth malocclusion can cause dentition malfunction and has aesthetic impact which can seriously affect an individual's self image. Orthodontic status depends on the dentition, the youngster and their parents' awareness of the need for treatment and cultural, racial and socio-economic conditions. In this survey we assessed the orthodontic status in soldiers in the IDF, whether they had needed treatment or had been treated, and reasons for not getting necessary treatment. Methods: Ninety-five consecutive soldiers examined at a general military dental clinic were enrolled. In addition to their general examination, they were evaluated by an orthodontist. Questionnaires concerning past orthodontic treatment were filled out. Results: The soldier's median age was 20.8. 48 were male & 47 female. Per history, 35% of the soldiers had been treated by an orthodontist. Per examination, 79% of the soldiers (75) had needed orthodontic treatment in the past. Of these, 31% (23/75) had received adequate treatment, 13% (10/75) had been treated but their treatment was found inadequate, and 56% (42/75) had not received treatment. The main reasons reported for not applying for necessary treatment were the dentist thought they didn't need it or didn't mention this treatment option (76%, 32/42), or the parents didn't think it necessary (29%). Other factors like fear of pain or aesthetic complications and financial considerations were less frequent. Conclusions: This survey presents a considerable gap from the desired state. Being based on a retrospective report and subjective impression, there is probably overrating of the dentists' role in not recommending orthodontic treatment. Further prospective study is needed to elaborate the actual reasons for lack of treatment. However, it is fair to assume that more understanding among dentists, both in diagnosis and in practicing health education is required. Intervention for raising the awareness among parents and children is also needed.
