Developing a protocol for use of a diffuse reflectance probe to detect oral cancer arising from oral potentially malignant disease

Objectives: We aim to produce a protocol for intra-oral use of diffuse reflectance spectroscopy (DRS) technology. We also aim to assess this protocol’s accuracy in reliable demarcation of oral squamous cell carcinoma (OSCC), oral potentially malignant disease (OPMD) and normal tissue, with a long-term goal of instantaneous diagnosis of early stage OSCC. We aim to disseminate any results and protocol in order to provide a template for future research and development of light-based diagnostic tools in the mouth.
Methods: A cross-sectional study was conducted with an initial optimization group of 10 patients used to obtain a set of discrete optical parameters for anatomical subsites. Recruitment of patients with a known histopathological diagnosis of (a) OPMD, (b) OSCC and (c) control groups followed. Readings of light reflectance from OPMD, OSCC, normal and control tissues were obtained by multiple examiners using set parameters
Results: A total of 66 patients were recruited. Data was recorded with respect to patient demographics, histopathological diagnosis and site of measurement. The primary outcome measure was reflectance spectra, assigned by group according to prior diagnosis. The measurements were found to be non-invasive and well-tolerated. Preliminary data analysis suggests our protocol produces spectra which are distinct by diagnosis and anatomical area and reproducible between patients.
Conclusions: The initial optimization stage was successful in extracting parameters for oral tissues. Early data demonstrates ability of our DRS protocol to distinguish OSCC, OPMD and normal. It is acceptable to patients. Further data gathering can now establish this protocol’s applicability in other settings.
