Fluoride Mouthrinse Use In Orthodontic Patients With Fixed Appliances
Objectives: (1)To assess if orthodontic patients wearing fixed appliances recall receiving advice regarding daily use of a fluoride mouthrinse and whether delivery of advice was recorded in the clinical notes; (2) To evaluate reported compliance with this advice.
Gold Standards (1) All patients should be advised verbally by the treating clinician about the daily use of a fluoride mouthrinse at the time of fixed appliance bond-up and this should be documented in the clinical notes. (2) All patients should report compliance with daily use of fluoride mouthrinse. Methods: From January 23rd 2023 to February 15th 2023, 100 consecutive patients wearing fixed appliances were invited to complete a pre-piloted paper questionnaire at their routine fixed appliance adjustment appointment. All recorded data were collated on a Microsoft Excel spreadsheet and analysed. Results: All invited patients participated (51 females; 49 males). The average time since bond up was 15.2 months. 72% of patients reported being advised by the treating clinician regarding daily use of a fluoride mouthrinse during treatment. The recommendation for daily use of a fluoride mouthrinse during fixed appliance treatment was recorded in 43% of clinical notes.
77% of patients reported using a fluoride mouthrinse with 24% reporting use once a day, 29% twice a day and 3% more than twice a day. 14% used a fluoride mouthrinse a few times per week. Of those who reported use of a fluoride mouthrinse, 53% did so after brushing. The concentration of fluoride in the mouthrinses used varied greatly between patients (0ppm to 450ppm). Conclusions: None of the Gold Standards were met.
Recommendations: Bond-up appointments should be highlighted on the computer system to flag the need for relevant patient information leaflets and verbal advice regarding daily use of a fluoride mouthrinse, both of which should be documented in the clinical notes.
Re-audit in one year after these changes have been implemented.
