Health science education for students should comply with the principles of the Humanistic Environment. Adhering to these principles, along with other didactic and clinical training courses, will make treatment more successfuland brings more satisfaction for patients. Referring to the available scientific resources and related researches, a questionnaire consisting of 67 questions was collected for evaluation from the literature. In addition, some other related themes were added to it. The questionnaire was submitted to 9 members of a panel of experts for a survey. The statistical methods of CVI and CVR were determined and its reliability was evaluated. Furthermore, The questionnaire was administered to 8 students for determining the Face Validity. Cronbach's alpha calculation was 0.808, using SPSS (v.#22) software. The Reliability Analysis was determined based upon 15 students? opinion. Eventually, a questionnaire containing 39 questions was finalized. The developed questionnaire consists of 7 discriminate sections: internal feelings, Rules and Regulation, Interpersonal Relations, Behavior Change, Administration, Commitment and Satisfaction, and Professional Ethics. The Interpersonal Relations and Professional Ethics showed the highest score of questions compared to other sections, while Rules and Regulation had the least. Due to cultural and social differences, a specific questionnaire should be designed for a unique community to define the appropriate Humanistic Environment criteria. These criteria must be implemented for satisfaction of patient and to achieve successful treatment outcome, along with regular clinical skills. The present developed questionnaire is a standardized tool for implementation, in order to evaluate the humanistic environment commitment of dental education setups.
