Relationship between Migraine and pre and post operative pain following root canal treatment

Objectives: one of the main concerns in dentistry is the pain related to teeth involved endodontically. Finding the etiologies of such pain and managing them subsequently is of utmost help to both patient and dentist.
Migraine pain is a common neurovascular headache that may refer pain to teeth due to possible similar neuronal network coverage as teeth.
The aim of this study was to evaluate the relationship between migraine headaches and pain related to root canal therapy, pre and post operatively.
Methods: In this case control study, 160 patients were enrolled sequentially into four groups of 40 by their attendance to the endodontics department of Azad University of Tehran, dental branch. Patients were categorized into having or not having migraine by completing a form, then the status and pain of the endodontically involved teeth were recorded. The patient was considered symptomatic if they reported their pain intensity 5 or more in a 10 cm Visual Analogue Scale (VAS). After the completion of the root canal therapy, post operative pain was evaluted at 6, 12, and 24 hours using the VAS. The data were analyzed using repeated measured ANOVA, Friedman, two-way ANOVA, Partial correlation and χ2 .
Results: Having migraine headache and symptomatic tooth was significantly higher in women (P<0.05). Symptomatic patients, without considering having migraine or not, reported higher pain intensity at 6 and 12 (P=0.082) and 24 hours (P=0.009) after root canal treatment. However, the symptomatic patients with migraine reported higher pain intensity comparing the asymptomatic migraine group at all time intervals (P=0.001). As a whole, analgesic consumption was higher in the symptomatic group (P=0.006). The pain intensity at different times had strong correlation with medication consumption (P=0.0001).
Conclusions: women presented more with migraine headache and/or symptomatic tooth.The pain intensity was higher in patients having migraine and a symptomatic endodontically involved tooth prior to root canal therapy. Symptomatic patients having migraine experienced the highest pain intensity at 6, 12, and 24 hours following root canal treatment.
