Evaluating the method and outcomes of miniature pulpotomy procedure in vital pulp therapy
Objectives: Objectives: The aim of this study was to evaluate the method and outcomes of miniature pulpotomy (MP) as an ultra-conservative treatment modality of vital pulp therapy(VPT).
Methods: Methods: An electronic search was performed using Google Scholar, PubMed and ScienceDirect based on PRISMA statement for systematic review and Meta-Analysis. Studies containing MP in VPT were revised. The reviewed articles were subjected to meta-analysis for quantifying the differences. The resulting studies were analyzed according to the MP indications and parameters comparing the outcomes of MP with other VPT techniques.
Results: Results: The research retrieved 226 unique citations from which 7 remained after considering the abstracts. As evidence suggests, MP was defined as the procedure with gentle/limited removal of the infected dentin chips/damaged pulp tissue especially the injured odontoblast cell layer after direct exposure of the pulp tissue that would not exceed ~1 mm; this treatment modality ensures a clean surgical wound and improved proximity/interaction of pulp covering agent to undifferentiated mesenchymal stem cells.
Conclusions: Conclusion: The ultimate goal of all treatment modalities of VPT of adults’ permanent dentition is preservation of the healthy portion of the pulp; Although there has been no universal agreement on the best treatment for cariously exposed vital pulps of permanent teeth.MP provides an conservative and biologically-based treatment that maintains pulpal vitality and shows the ability to resolve emerging apical pathosis . Although more clinical trials with larger sample sizes and longer-term follow-ups may be required, using different bioceramic materials for MP may be considered an alternative option to conventional RCT in symptomatic vital permanent teeth diagnosed with irreversible pulpitis.
