EROSIVE TOOTH WEAR IN PRESCHOOL CHILDREN OF MONTEVIDEO – URUGUAY: CROSS- SECTIONAL STUDY

Objectives: To assess the prevalence, extent, severity and distribution of erosive tooth wear (ETW) in preschool children of Montevideo - Uruguay and its association with risk indicators.
Methods: A cross-sectional population-based study was carried out in preschool children from the department of Montevideo. A representative sample was selected, stratified by conglomerate in two stages. In a first stage, 38 (20 public and 18 private) preschool centers were drawn. In a second stage, 20 children from each center were drawn. A structured questionnaire was sent to the parents, investigating sociodemographic, biological and behavioral variables. Clinical examinations were performed by two calibrated examiners (Kappa ≥ 0.7). ETW was reported through the Basic Erosive Wear Examination index (B.E.W.E). Univariated and multivariated logistic regression analysis were performed to establish associations between independent variables and ETW.
Results: 614 preschool children were examined (48.4% from private initial centers and 51.6% from public centers). Prevalence ETW: 57.6% [95%CI=52.7-62.5] B.E.W.E.≥1. Severe ETW: 8.7% [95%CI 6.0-11.3] B.E.W.E.≥2. ETW extension (mean of affected surfaces): 4.8 [95%CI=4.3-5.3] and 0.37 [95%CI=0.21-0.52] B.E.W.E. ≥1 and ≥2 respectively. The most affected surfaces were palatal of upper incisors. It was found a significant association between ETW B.E.W.E.≥1, children who attended private dental clinics OR=3.07 [95%IC 1.51-6.28] and a negative association with those who consumed yogurt frequently OR=0.53 [95%IC=0.30-0.92]. A significant association between ETW B.E.W.E.≥2 was found with children from high socioeconomic status OR=2.91 [95%IC=1.03-8.21], with those who consumed Ades® juice frequently OR=3.06 [95%IC=1.44-6.64], and a negative association was found with those who consumed yogurt frequently OR=0.34 [95%IC=0.15-0.74].
Conclusions: ETW prevalence found among Uruguayan preschool children was high, being palatal of the upper incisors the most affected surface. It was found a significant association with high socioeconomic status, frequent consumption of Ades® juice, being the frequent consumption of yogurt a protective factor.
