Multidisciplinary frontiers of periodontopathic compromised case in esthetic zone

Objectives: to evaluate the combined prosthetic-periodontal approach in management of periodontally compromised teeth in the esthetic zone using modified FDI criteria.
Methods: 32 years old female with controlled medical condition presented with old badly constructed cantilever bridge replacing missed upper left central incisor 13 years ago with persistent gingival inflammation related to upper right central. The patient was complaining from unpleasing appearance of her teeth. Also, the patient wants to replace missed upper left first premolar and upper right second premolar. clinical and radiographic examination” CBCT” revealed violation of biologic width of upper right central incisor. Removal of old bridge was performed. Periodontal surgery was done to re-establish the biological width and after healing period of three months, final preparation and impression was taken. After final insertion of the permanent restorations fabricated from pressable glass ceramics and monolithic zirconia, the case was evaluated regarding gingival and periodontal health associated with indirect dental restorations using modified FDI criteria.
Results: The evaluated FDI criteria was excellent regarding Gingival health in terms of Gingival index, sulcus bleeding index, modified sulcus bleeding index, papillary bleeding index, bleeding on probing or brushing. Excellent Periodontal health conditions, measurement of pocket depth with no bleeding on probing were observed. For the prosthetic evaluation regarding Functional properties “marginal adaptation, proximal contact point”, form and contour and occlusion and wear scored excellent as confirmed clinically and radiographically. For Biological properties, there was no postoperative hypersensitivity and good pulp status. As for esthetic properties, excellent colour match was recorded.
Conclusions: periodontal surgery combined with prosthetic treatment represent optimum multidisciplinary frontiers and can meet patient expectation from dental treatment as this approach provides the patient with integrated restorations biologically, functionally, and esthetically.
