Maxilla Setback By Lefort I Osteotomy for Correction Maxillary Prognathism
[Abstract] Objective: To study the application of total maxilla setback by Le Fort I osteotomy for correction of maxillary prognathism deformity. Methods: Twelve patients with maxillary prognathism deformity with skeletal Class II malocclusion,5 male and 7 female from 18 to 31 years old,were undergone combined orthodontic and orthognathic surgery treatment.Ten of them are maxillary prognathism with mandibular deficiency and two of them are only maxillary prognathism.All cases were undergone total maxilla setback by Le Fort I osteotomy,10 cases were simultaneously undergone bilateral sagittal split ramus osteotomy to advance their mandibles and 4 cases were simultaneously undergone genioplasty to advance their chins.The key procedures of total maxilla setback by Le Fort I osteotomy are to anatomize descending palatine arteries carefully from great palatine canals,to cut off part of lateral pterygoid plates,to cut off the maxillary tuberosities and to take out the third molars. Results: All wounds healed well,there was no serve complication. The clinical follow-up period ranged from 12 to 18 months.After the combined orthodontic and orthognathic surgery treatment,all patients regained satisfactory face appearances and function. Conclusion: Total maxilla setback by Le Fort I osteotomy is an effective way to cure part of patients with maxillary prognathism deformity with skeletal Class II malocclusion.
