In China, It often can cause a lot of problems such as space-change, translocation of the neighbor-teeth and the following permanent teeth, if we don't do some effective space-contain after the primary teeth which have serious problems such as periapical inflammation, periapical osteolysis involving the teeth germs, were moved just because of lack of oral-care knowledge popularizing, pediatric professionals and the cooperation of parents. So ,it is necessary to find a method of primary teeth-contain ,which is fit for our country. Objective: to observe the clinical effects of the root canal therapy to the primary molars, which would have been extracted caused by periapical diseases according to the clinical standard. Method: to do the root canal fillings-therapy with the Vitapex paste to the 22 primary molars ,which have periapical inflammation, periapical osteolysis involving the teeth germs, and check up regularly. Results: the periapical diseases of 21 primary molars have been well controlled after the root canal therapy, the osteolysis, which happened under the roots of the primary molars and to the teeth germs involved, are reduced effectively. Only one case failed. Conclusion: it is not appropriate to take it as an only standard that to extract a primary molar just because of its periapical disease involved the tooth germ below. To decide whether to extract such a primary molar should consider the age of patient, clinical syndrome, calcifying of the tooth germ below and so on, and then evaluate all the situation in detail. To some cases, it is not necessary to remove the primary molar.
