 The relationship of maternal care and anxiety with chronic periodontitis

Relación de apego materno y ansiedad con periodontitis crónica. Estudio transversal. Quintero AJ*, Calderón R, Chaparro A, Sanz A, Ventura-Juncá F.

Objetivos: Evaluar el efecto del apego materno y de la ansiedad en la severidad de la periodontitis.

Métodos: Estudio transversal. Se seleccionaron de forma aleatoria 106 pacientes. A todos ellos se les entregó dos autorreportes: Parental Bonding Instrument (PBI) para evaluar apego materno, y Escala de Ansiedad de Hamilton (HAS) para medir nivel de ansiedad. Se realizó examen periodontal midiendo índice gingival (GI), profundidad al sondaje (PPD) y nivel de inserción clínica (CAL) . Los pacientes fueron clasificados en periodontitis severa, moderada y leve o sin periodontitis. Los antecedentes fueron analizados con el software STATA ©. Se utilizó regresión lineal múltiple para correlacionar los indicadores de riesgo con el sangramiento al sondaje y pérdida de piezas dentarias, Regresión Logística Ordenada para correlacionar indicadores de riesgo con el diagnóstico periodontal, y Regresión Logística Simple para variables dicotómicas.

Resultados: Se evaluaron 106 pacientes. 15 pacientes presentaron "Periodontitis Leve o Ausente", 53 presentaron "Periodontitis Moderada" y 38 presentaron "Periodontitis Severa". Se analizaron los resultados de las encuestas, HAS y PBI y se los correlacionó con los resultados de severidad de periodontitis, presencia de sangrado al sondaje y pérdida de piezas dentarias. Existe correlación positiva entre cuidado maternal deficiente y diagnóstico de Periodontitis, con un OR de 5.323 (P>|z|=0.007). El puntaje promedio de HAS aumenta en la medida que aumenta la severidad de la Periodontitis. El puntaje PBI disminuye ante tal evento.

Conclusiones: En la población estudiada, los pacientes con historia de cuidado maternal deficiente presentan un mayor riesgo para padecer Periodontitis Severa. Además, estos pacientes exhiben un mayor riesgo de presentar un elevado nivel de ansiedad

Objectives: Evaluate the effects of parental bonding, and anxiety in severity of periodontitis . Methods: In this cross-sectional study, 106 subjects were evaluated. Maternal care were assessed using the Parental Bonding Instrument (PBI) whereas anxiety were evaluated using the Hamilton Anxiety Scale (HAS). Clinical measures such as PI, gingival index (GI), periodontal probing depth (PPD) and clinical attachment level (CAL) were collected. Patient's medical history and socioeconomic data were also recorded. The patients were classified according their periodontal status in severe, moderate and no periodontitis. The records were analyzed with software STATA ©. Multiple linear regression was used to correlate the risk indicators with bleeding on probing and tooth loss; ordered logistic regression to correlate with periodontal diagnosis and simple logistic regression to correlate the data into dichotomous variables. Results: 106 patients were evaluated. 15 patients had “mild or absent periodontitis”, 53 patients had “moderate periodontitis” and 38 patients had “severe periodontitis”. We analyzed the surveys results, HAS and PBI, and the correlation with severity of periodontitis, presence of bleeding on probing and tooth loss. There is a positive correlation between poor maternal care and severity of periodontitis, with an OR of 5,32 (P>[Z] = 0,007). The average score of HAS increases with increasing the severity of periodontitis. The score of PBI decreases to such an event. Conclusion: In the study population, patients with a history of poor maternal care are at increased risk for developing severe periodontitis. In addition, these patients exhibit a higher risk of having a high level of anxiety.
