Dental Implant Therapy In Ligneous Periodontitis Patients: 5 years follow-up

Objective: Ligneous periodontitis is an analogue lesion of ligneous conjunctivitis, which is characterized with plasminogen deficiency and conjunctival pseudomembraneous lesions. The disease is characterized by membranous gingival enlargement due to an accumulation of fibrin deposits and periodontal tissue destruction that leads to rapid bone loss. Different treatment modalities with limited success have been tried to cure these patients, including surgical excision, curettage, antbiotics, corticosteroids and some other chemotherapeutic agents.

Method: This case report presents the clinical and histopathological evaluation of dental implant therapy of two siblings with ligneous periodontitis and 5 years follow-up.

Result: Both of the patients indicated membranous gingival enlargements and mild horizontal bone loss nearby the implants. Ligneous peri-implantitis lesions were treated by initial mechanical therapy and resective surgery. Histological evaluation revealed epithelial degeneration, dense inflammation and fibrinoid accumulation in connective tissue adjacent to epitelium.

Conclusion:

Ligneous periodontitis patients may be treated using dental implants succesfully. However, increased risk of peri-implantitis should be taken into account and thorough maintenance is necessary.
