Long Term Results Following Surgery with PRP and NBM

BACKGROUND: Regenerative periodontal surgery utilizing a combination of a natural bone mineral (NBM) and platelet-rich plasma (PRP) has been shown to enhance the outcomes of regenerative surgery. At present, the long-term effects of this therapy are unknown. AIM: To evaluate clinically the long term results following treatment of deep intrabony defects with either NBM+PRP or NBM alone. MATERIAL AND METHODS: Thirty patients suffering from advanced chronic periodontitis, and each of whom displayed one advanced intrabony defect were randomly treated with either NBM+PRP (test) or NBM (control). Clinical parameters were evaluated at baseline , at 1 year and 5 years after treatment. The primary outcome variable was clinical attachment level (CAL). RESULTS: The test sites showed a reduction in mean probing depth (PD) from baseline 8.17 ± 1.03 mm to 3.17 ± 1.27 mm at 1 year and to 2.83 ± 2.08 at 5 years, respectively. In the control group mean PD was reduced from baseline 8.92 ± 2.07 mm to 3.50 ± 1.31 mm at 1 year and to 3.50 ± 1.62 at 5 years. In the test group mean CAL changed from baseline 9.67 ± 1.92 mm to 4.08 ± 1.38 mm at 1 year and 3.75 ± 2.26 mm at 5 years, respectively. In the control group mean CAL changed from baseline 9.83 ± 2.76 mm to 4.58 ± 1.83 mm at 1 year to 4.42 ± 1.56 at five years. There were always statistically significant differences between baseline and one year data. No statistically significant differences in any of the investigated parameters were observed within groups and between the two groups at 1 and 5 years. CONCLUSIONS: The present results have shown that the clinical improvements obtained with both treatments were stable over a period of 5 years.
