Desquamative Gingivitis: Dental and Medical Considerations.

Objectives: The aim of this study was to describe the demographic, clinical and histological features of 200 patients with Desquamative Gingivitis (DG), who attended the Clinic of Oral Medicine and Oral Pathology, School of Dentistry of Aristotle University of Thessaloniki.
Methods: The parameters studied were sex, age of patients, the underlying etiology of DG, intercurrent medical problems and addictions (smoking, alcohol abuse). Histological examination was performed on a smaller sample population (110 patients). Also was recorded in detail the extent and localization of lesions on the gingival tissues, the proximity to dental works and the co-existence of extraoral lesions.
Results: 158 patients of the population were female (79%) and 42 were men (21%), giving the sample of female/male ratio of 3:1. The average age of patients was determined 57.97 years with a standard deviation of ±15,45 years. Oral Lichen Planus was the most common underlying cause of DG in 55.50% of cases, followed of mucous membrane pemphigoid in 29,50% of cases. DG was observed primarily in localized form and found to occur more frequently in the maxilla than in mandible and labial gingival tissues compared to palatal gingiva. There was extra-oral involvement in 14% of patients, with skin (10%) and genital mucosa (4%) to be the most common sites of attack. In most cases, DG proved to occur adjacent to dental works.
Conclusions: DG is a descriptive term but not an independent disease entity, which constitutes clinical manifestations of several mucocutaneous disorders and systemic conditions. The management of DG comprising the correct diagnosis establishment in order to be given the appropriate medical treatment for the underlying disease, improvement of oral hygiene, minimization of irritating factors, system of regular follow-ups and the pharmacological treatment with topical or systemic immuno-suppressants according to the extent of the lesions.
