Remote Pain in TMD Patients Is Related to COMT Polymorphism

Objectives: To evaluate the relationship of catechol-O-methyltransferase (COMT) gene polymorphisms with remote pain in patients with temporomandibular disorders (TMD).
Methods: Ninety patients with pain-related TMD diagnosis and 92 matched controls were assessed for the presence of TMD by Research Diagnostic Criteria for TMD. Information about remote pain in the prior 6 months (susceptibility to painful symptoms in other parts of the body; head, chest, muscles and back) was collected from Symptom Report Questionnaire. DNA extracted from peripheral blood was genotyped for three COMT SNPs (rs4680, rs6269, and rs165774) by Real-time Taq-Man method. The relation between COMT and clinical variables was determined by Kruskal-Wallis and Mann-Whitney tests.
Results: A large majority of TMD patients (82.2%) had regular headaches and about half of them reported body muscle pain (51.1%), a prevalence that was significantly higher than in controls (p=0.006, p=0.017, respectively). Homozygous genotype AA for rs6269 polymorphism was related to lower prevalence of body muscle pain (p=0.013) and less painful places (p=0.020) compared to heterozygous AG genotype. No relation for rs4680 and r165774 polymorphisms was observed.

Conclusions: TMD patients caring AA genotype of rs6269 polymorphism present lower prevalence of body muscle pain and less remote pain. Studied rs6269 variant might be involved in the individual susceptibility to pain in general among TMD patients.
