Digital (CAD/CAM) Dentures: Comparison of 3 Different Workflow Options

Objectives: To compare 3 different workflows for fabricating CAD/CAM removable dentures concerning clinical quality after placement.
Methods: 30 edentulous patients were treated with removable digital designed (Digital Denture Professional, Ivoclar Digital) and manufactured complete dentures (upper + lower jaw; n=60) with 3different workflows (10patients per group, randomly distributed; 2operators): group1=3-step-workflow milled dentures, group2=4-step-workflow milled dentures, group3=4-step-workflow IvoBase dentures (workflow details / materials=table1).
After incorporation dentures were evaluated (score 1-4) concerning aesthetic, fit/stability, occlusion, pressure marks and patient`s satisfaction (table2).
Results: All dentures of group1-3 were scored “very good” for aesthetic and patient`s satisfaction (table3). Concerning fit/stability 100% of cases (group1-3) were rated as “very good”, 12 cases (40%) even showed suction effect for upper and lower denture. Final occlusion for all dentures was rated as “very good”. 11 cases (37%) needed no occlusal adjustment after incorporation.
Pressure marks occurred in 26 cases (87%). No statistic difference for group 1-3. The overall number of isolated small pressure marks ranged from 0 (13%) to 6 (3.3%). The number of appointments (groups 1-3) for adjustment of occlusion and denture bases ranged from 1 to 5, whereby in 63% of the cases adjustments were finished after 2appointments. The majority of pressure marks occurred in contact to buccal (31%) and lingual (28%) border of lower dentures (no statistic difference for group 1-3).
Conclusions: All workflows are perfectly indicated for fabricating CAD/CAM removable dentures. Dentures quality at placement was not influenced by bite registration method. Moreover digital workflows have a big advantage: all information like aesthetic lines, lip support and occlusal plane are saved and can be used as control during denture design step. The predictability for dentures` aesthetic is enhanced. To check dentures` quality a tryin (groups 2+3) is possible, but not mandatory. Accurate fit of dentures is also a result of CAD/CAM technology because material expansion/shrinkage of gypsum/PMMA e.g. are eluded. The appearance of pressure marks mainly at border of lower dentures is related to individual patient factors and not to digital workflows.
