A Brief Intervention for Patients with Alcohol Related Facial Injury

Introduction and Objectives: The association between alcohol and violence is well known. The incidence of alcohol related facial injury in the UK has risen markedly over the past 20 years. The aim of this study was to determine the effectiveness of two different brief interventions on drinking patterns in patients who had sustained an alcohol related facial injury.

Methods: AUDIT (Alcohol Use Disorders Identification Test) scoring was used in three Oral and Maxillofacial Surgery (OMFS) trauma clinics to determine which patients might benefit from brief intervention. Patients with AUDIT scores > 8 (i.e. the cut-off point for hazardous drinking)were randomly assigned to receive either a motivational interview by a trained nurse or a leaflet on alcohol abuse. Patients were followed up at 3 and 12 months to assess the effect of each intervention on their drinking patterns. The 3 months results are presented.

Results: 249 patients underwent initial screening, 196 had an AUDIT score > 8 (78.7%). Of these 87% were male and 40% were aged 20-30 years. 70% had been drinking at the time of their injury and 20% had a previous alcohol related injury. 52% attended for follow-up at 3 months and it was noted that the motivational interview group had significantly fewer drinking days (P> 0.05)than the leaflet group.

Conclusions: Motivational interviewing for patients with alcohol related facial injury is possible in a busy OMFS clinic and is effective in reducing alcohol intake.
