Comparison Between Estheticone Abutment and 30º Angled Abutment in Premaxilla

Objectives: The purpose of this study was to compare the use of a straight abutment (Estheticone) with a 30 degree angled abutment in the load transmission between the components and the surrounding bone in the premaxilla region. Methods: Using a digital tomographic image of the pre-maxilla from a real clinical study, non-linear analysis through the method of two-dimensional finite element was used to demonstrate the stress distribution on the components of osseointegrated implant systems and respective crowns, as well as on the surrounding bone tissue. To compare the load distribution, when straight and angled abutments were used, a combined load of 112N (100N in axial direction and 50N in horizontal direction) was applied on the incisal third of prosthetic restorations according to the following conditions: (1) 3.75mm osseointegrated implant and metalloceramic crown in straight abutments Estheticone (Nobel Biocare®, Gothenburg, Sweden) and (2) 3.75mm osseointegrated implant and metalloceramic crown in 30 degree angled abutments (Nobel Biocare®, Gothenburg, Sweden). Results: The results showed greater localised stress concentration on the contact areas of the gold and the abutment screws of the angled set when compared to the screws of the straight set. A high stress concentration on the implant neck was observed for both straight and angled sets, although the stress concentration was 14% higher for the straight set. Conclusions: The stress distribution and related strains in the bone tissue were very similar for both straight and angled configurations.
