BENIGN PENPHIGOID OF MEMBRANA MUCOSAL MEMBRANE (PBMM): TWO CASE REPORTS WITH INAPROPIATED DIAGNOSIS
Introduction: PBMM or cicatricial penphigoid is an autoimmun desease with subepithelial blisters evolution that involved oral mucosa and eyes. Diagnostic seems to be problematic because poor immunologic evaluation, most of the time. It can be presented as descamativ gingivitis wich is persistent and limited to some adherent gíngival areas (similar to OLP, pènphigo vulgar, psoriasis and discoid eritemathous Lupus), .Objective: to confirm criterial diagnosis aplying to two case reports. Material and Methods ; by clinic observation , histology , immunohistochemical and immunofluorescence analyses there were studied two patiens, masculin ad femenin, with another autoimmune desease history and previous diagnòstics of oral lesions. Case 1 (women, 64years old, with reumathoid artritis and blistern localized on lips, oral mucosa ,both sides and palate. Previously there were diagnosticated as recidivant afthous ulcers.Caso 2, man , surgeon, wth oral liquen (previous diagnose) , and ignored descamative gingivitis .Some biopsies were performed o both cases.. Results: 1- By histology . diagnostic of subepithelial blisterns was confirm on both cases. .. 2- Case 1: hihg IgG y C3 ,reumatogram and colagenogram + ( reactive C protein ).Case 2: subepitelial blistern and abundant leucocitary infiltration was observed. Immunofluorescence was negative and Laminin immunomarcation was positive. . Antinuclear antibodies (ANCA) +. Immunofluorescence negative. Conclusions: 1- It were confirmed the histoligical patrons of concense for PBMM . 2- Inmunofluorescence was negative on both cases because lesions were on first steps of illnes .In that case it was taken an alternative parameter as Laminin which was +..3- Its important diagnostic in relationship with profession on the second case. were ofthalmologic evaluation was indicated as prevention.
