A Prototype Positioning System To Train Students in Crown Preparation

Objectives: Conventional dental teaching centres on demonstration and rehearsal of fundamental procedures first by simulation, thus allowing students to master the required skills for tooth restoration. Advances in technology have the potential to improve training outcomes and reduce training time. In this preliminary study, we test a novel visual real-time feedback tool, utilising Augmented Reality (AR) technology. Our objective is to determine whether using visual real-time feedback from a prototype positioning system for intraoperative use (PIP) aids dental students in reproducing tooth preparation shapes. The positioning system consists of a sensor attached to the handpiece, which relays a visual representation of bur angulation in relation to a user-set axis of insertion.
Methods: Thirteen dental students from UWA School of Dentistry (from years 2, 3 and 4) prepared standardised shapes on premolar and molar FRASACO teeth, using 3D-printed models as a reference, and standard existing setup for traditional operative techniques training. Each participant completed four preparations: two without AR feedback and two with guidance from the AR tool. Prepared teeth were then 3D scanned and compared with the reference models in terms of axial angulation.
Results: Quantitative data showed no statistically significant improvement in preparation dimensions in this pilot study. However, the data indicates that fewer undercuts were prepared using the PIP compared with no feedback. These preliminary results indicate that further familiarization with the device is needed for students to maximise the advantage PIP can provide. Qualitative feedback provided about the suitability of the device has led to improvements in stability and drift of the sensor
Conclusions: Future studies are required to test the improved device and provide an extended familiarization period, however this device has the potential to have a significant impact globally in both tertiary dental training programs and in operative techniques for clinical dental practice.
