Analysis of Tooth Mineral Density of Hypomineralised First Permanent Molars

Objectives: This study sought to evaluate (i) variations in tooth mineral density (TMD) for the different lesion types (brown, yellow/creamy, and white), and (ii) mineralisation potential of silver diamine fluoride (SDF) on hypomineralised first permanent molars (FPM)
Methods: Eighteen extracted FPM and six clinically sound FPM were analysed using Micro-CT. The teeth were treated with SDF and subsequently immersed in remineralising solution for 14 days. The mineral content of the hypomineralised lesions was assessed before- and after-treatment and compared to sound enamel.
Results: The average TMD for brown, yellow/creamy, white and unaffected enamel was 1.79g/cm3, 2.21g/cm3, 2.43g/cm3 and 2.46g/cm3 respectively. Brown and yellow/creamy lesions exhibited a statistically significant difference when compared to white lesions and unaffected enamel (p<0.05). However, no statistical difference was evident in TMD between white lesions and unaffected enamel. There was a significant increase in the TMD following the SDF placement. Polarized light microscopy assessment illustrated a deeper penetration of SDF in severely hypomineralised FPM enamel lesions.
Conclusions: TMD was lowest for brown lesions followed by yellow/creamy lesions while the TMD for white lesions was similar to unaffected enamel. SDF was effective in mineralizing enamel lesions in hypomineralised FPM.
