Immediate (Type 1) v/s Early (Type 2) Single Implant Placement in the Anterior Maxilla: A Prospective Comparative Study

Objectives: This prospective study evaluated the impact of the timing of implant placement (immediate v/s early) on horizontal ridge dimensional changes, as well as clinical and aesthetic outcomes, associated with single tooth replacement in the anterior maxilla.
Methods: Single tooth implants were placed in the anterior maxilla in 30 patients either immediately after extraction (Group A, n = 15) or after a healing period of 4 – 8 weeks (Group B, n = 15). Study models were obtained before extraction (T0), at implant placement (T1), at the insertion of a definitive crown (T2), and 1-year thereafter (T3). Horizontal ridge dimensional changes were measured by superimposing the optical scans of the study models from different time-points. Radiographs and photographs were used to evaluate changes in marginal bone levels and soft tissue recession. Aesthetic evaluation was carried out using the Pink and White Esthetic Score (PES and WES) indices. A subjective questionnaire was used to measure patient-centered outcomes.

Results: A 100% implant survival rate was observed in both groups. Analysis showed a labial horizontal tissue dimensional change of 0.61 mm and 0.72 mm from T0 –T3 in Groups A and B, respectively (P = 0.64). Comparable PES values were observed in both groups (A v/s B: 9.40 v/s 9.27; P = 0.83) after the 1-year follow up period. No incidence of advanced mid-labial mucosal recession (>1mm) was observed between definitive crown insertion to 1-year follow-up in either group. No significant changes were observed for all other measured variables at different time-points.

Conclusions: Single tooth implants in the anterior maxilla can achieve comparable satisfactory clinical and aesthetic outcomes when placed either immediately (Type 1) or after a period of 4-8 weeks following tooth extraction (Type 2). No difference in labial horizontal ridge resorption was identified between the two placement protocols.
