Governance in the regionalized healthcare network and its influence on oral health: analysis from decision-making space
Objectives: Healthcare network structuring needs to be grounded in a participative governance system through negotiation and building of consensus among stakeholders. The study aims to analyze the governance system undertaken to structure regionalized healthcare network through a decision-making space known as State Manager Committee in the state of Santa Catarina, Brazil, and focused on investigating the oral health care system.
Methods: It was performed as a qualitative, exploratory and documental study, based on an analytical framework for governance, through identifying four main analytical units: actors, norms, processes and nodal points. Secondary data was gathered from meetings reports performed from January of 2011 to December of 2015.
Results: The elements analyzed show deficiencies in the governance process conducted within the decision-making space investigated, mainly in issues regarding the lack of structuring of the oral health care services in a regionalized form and through a network perspective. The oral health care structuring remains dissociated from the building of a healthcare network. The effect of the current governance process is present in the expansion of public dental services but there is still no integration among the services. The relationship established by the stakeholders that were identified in this study showed that the managers mostly follow higher level recommendations, from state secretaries or national level, and there are few examples of negotiation regarding the local demands.
Conclusions: Therefore, the policy-making process to structure oral health care in this state is limited and demonstrates a centralized stance on the agenda of local/regional managers.
