The Effect of Starting Tooth Shade on Bleach Treatment Results

Objectives: To compare the color change of natural extracted teeth of different baseline shades (C, D) following treatment with one of two bleaching products with and without the product light-emitting diode (LED). Shade C falls in the greyish color class while shade D falls in the reddish-grey color classes, both known for being difficult to whiten.
Methods: Extracted human molars were mounted in stone arch forms (6 teeth/arch) and treated with SNOW Teeth Whitening Serum (Phoenix, AZ) or Crest Whitening Emulsions (Greensboro, NC) with and without the associated LED-tray. Baseline shades were recorded (Easyshade V, VITA, Yorba Linda, CA). Arches were randomly divided into four groups: 1) SNOW gel with light; 2) SNOW gel, no light; 3) Crest gel with light; 4) Crest gel, no light. Samples were exposed to 19 treatments. L*a*b* values were recorded following treatment and ΔE calculated (CIE 2000). ANOVA was performed testing for an interaction between the two grey-tone tooth color classes (C, D), product (Crest, Snow) and treatment (Gel-only, Gel with Lighted Tray) on total ΔE over the 19 treatments (α=0.05).
Results: Shapiro normality tests and visual inspection confirmed some subgroups were not normally distributed, so data were log-transformed prior to analysis. Results show a significant interaction between product and treatment (p-value=0.026, F=5.424). The results of this trial study did not reach significance, however the results were strongly indicative of different effects based on tooth starting shade (p-value = 0.057, F=3.856). Both color classes showed greater shade/color change for the Gel with Lighted tray, but the C color class showed stronger shade change in the Crest gel-only group compared to the D color class (Fig. 1).
Conclusions: Snow products produced consistent results for both baseline color classes. While Crest with the LED-tray was comparable, Crest gel-only resulted in greater color change in class C teeth (greyish).
