MicroCT Assessment of Dentin Demineralization Prevention by SDF and SDF-KI

Objectives: Silver Diamine Fluoride (SDF) has been used to prevent or arrest root caries. Potassium Iodide (KI) creates a white precipitate with SDF that reduces the discoloration, but its effects on SDF efficacy is unknown. This study compared preventive effects of SDF and SDF-KI on root dentin demineralization compared to fluoride varnish using microfocus X-ray computed tomography (microCT) methodology.
Methods: Bovine incisor root dentin blocks (3x4x3 mm3) were embedded in resin. The surface was then polished and covered by nail varnish leaving exposed sections. The specimens were then subjected to pretreatment in 4 groups (n=8); Control: deionized water (DIW); F-Varnish: 5% sodium fluoride varnish (Duralor, Medicom); SDF: 38% Saforide (Bee Brand Medico Dental); SDF-KI: SDF followed by KI (Sigma-Aldrich) saturated solution in DIW. Dentin surface was then thoroughly rinsed with DIW and subjected to 8 cycles of demineralization solution (pH 5) for 16 hours and remineralization in artificial saliva (pH 7) for 8 hours. Specimens were then scanned using microCT (North Star Imaging X5000) at 8.3 µm resolution and 12 minutes scan time. The 3D images were analyzed in Amira (VSG FEI) to calculate lesion depth (LD) and mineral loss (ΔZ) for each specimen. Data were analyzed by one-way ANOVA with pair comparisons (alpha=0.05).
Results: The LD and ΔZ data are presented in figure. There was a statistically significant difference between Control and all three other groups in both LD and ΔZ (p<0.05). While F-Varnish showed the smallest LD and ΔZ values, pair-comparisons with Bonferroni correction suggested that there was no significant difference among F-Varnish, SDF and SDF-KI (p>0.05). A radiopaque dense surface layer was observed in microCT images of SDF and SDF-KI, which was attributed to silver penetration into dentin surface.
Conclusions: Single application of F-Varnish, SDF and SDF-KI showed comparable preventive effects against root dentin demineralization. Application of KI did not influence anti-demineralization properties of SDF.
