The Effect of SnF2 Dentifrice on Virulence of Subgingival Plaque
Objectives: Antimicrobial toothpastes containing stannous fluoride has been shown to reduce virulence factors of dental plaque (AADR abstract 0095, 2016). The latter are highly pro-inflammatory and suspects of culprits in gingivitis. We conducted a 2-month clinical study to determine whether novel SnF2 dentifrice could decrease microbial toxicity of the subgingival plaque, and as an effect reduce the clinical manifestations of gingivitis.
Methods: Ninety-nine healthy subjects with gingivitis completed the study. Gingivitis was evaluated using Modified Gingival Index (MGI) and Gingival Bleeding Index (GBI). At Baseline, all subjects received Crest Pro Health dentifrice (0.454% SnF2) and a regular manual brush (Oral-B Indicator P35) for 8 week home use. Subgingival plaques were collected using curettes from three bleeding sites per participant at Baseline and Week 8, and tested in hTLR2 cell assay. The hTLR2 cells were purchased from Invivogen (San Diego, CA). Cells were exposed to the processed subgingival plaque samples and expression of hTLR2 reporter gene (secreted embryonic alkaline phosphatase, SEAP) was quantified with a commercially available kit.
Results: Treatment with SnF2 dentifrice resulted in significant (p<0.01) improvements in gingival bleeding (52% reduction as measured by GBI) and inflammation (12% reduction as measured by MGI) at Week 8. For the hTLR2 reporter gene assay SEAP activities were higher at Baseline (mean 68.77 (SE 7.42) TLR2 relative luminescence units, RLU), and reduced to mean 46.81 (SE 5.77) RLU at Week 8 (p=0.0004).
Conclusions: Novel SnF2 dentifrice reduced number of bleeding sites and gingival inflammation. Consistent with clinical observations, virulence of the subgingival plaque, measured by hTLR2 receptor-mediated responses, was also decreased significantly from Baseline to Week 8.
