Association of Mandibular Angle With Temporomandibular Joint Disorders
Objectives: Temporomandibular joint (TMJ) disorders (TMD) are a musculoskeletal symptom that can result from multifactorial causes. It has been noted that patients with hyperdivergent facial pattern show more signs of TMD. Thus, the objective of this study was to examine whether the mandibular angle is associated with TMD that is accounted for alteration of TMJ condyle bone mineral density (BMD).
Methods: Following approved IRB protocol, cone beam computed tomography (CBCT) images of TMD patients (4 males and 10 females, 18 to 36 years) and age matched healthy patients (3 males and 9 females) were retrospectively analyzed. Right and left mandibular condyles were digitally dissected from top surface to the occlusal plane. The BMD was measured by obtaining total CT attenuation values in the condyle. The 3D CBCT images were converted to lateral cephalograms to measure angle of mandible formed by the smaller value made with Co-Go line and Go-Pog (Pogonion) line. Pearson correlations were performed to test the associations between BMD and mandibular angle for TMD and healthy control groups (Significance, p<0.05).
Results: Both right and left condylar BMDs of TMD patients were significantly lower than the control group (p<0.001). The TMD patients had significant positive correlations of the mandibular angle with right and left condylar BMDs (p<0.015) while they were not significant for the control group (p>0.124).
Conclusions: The TMJ condylar BMD can be altered by abnormal bone modeling and remodeling at the TMD patients. The current findings suggest that angle of mandible is likely introduced as a potential risk factor of TMD.
