Effect of Bleach Lamp Using Combination In-office and Take-home Peroxides

Objectives:

To compare effect of 6% hydrogen peroxide (HP) in-office plus 3 days take home 16% carbamide peroxide (CP).
Methods:
Seventy-seven patients (n=308 teeth) were treated chairside with 6%HP with and without light (Zoom WhiteSpeed) and treated for 3 nights with 16%CP (NiteWhite) overnight starting at visit 3. Visual (VITA Bleachedguide 3D-Master, BG) and instrumental (Vita Easyshade) measurements were taken after in-office and post combination treatment. Color differences (ΔE*) were calculated; ΔE*=2.7 represents the 50:50% acceptability threshold(AT) and ΔE*=1.2 represents 50:50% perceptibility threshold(PT). A paired t-test at the 0.05 alpha level was performed to compare light vs no light means within subjects at each time point for instrumental and visual data. A survey of tooth and gingival sensitivity was completed.
Results: Means(s.d.) for time period comparisons are listed below.


The results shows significant differences between light and no light groups for SGU at each time period (p=0.0001, 0.0089, 0.0148, respectively) while there were no significant differences at each time period with respect to ΔE*.


The greatest majority of subjects reported no tooth or gingival sensitivity for in-office while some mild sensitivity was reported for overnight treatment.
Conclusions: The use of a bleaching lamp showed improved visual and instrument color changes compared to no light at each time period. An additional 3 nights of treatment with 16% CP after chairside whitening was effective in preventing rebound.
