Characteristics of Patients Presenting for Gummy Smile Treatment
Objectives: This study aims to examine the pre-operative clinical characteristics and satisfaction of patients presenting for gummy smile treatment. Methods: Patients requesting treatment for gummy smile and diagnosed with hypermobile upper lip were recruited. Inclusion criteria: adult (≥18 yrs); non-smoker; systemically and periodontally healthy; maxillary anterior teeth of normal dimensions; normal length upper lip; lip mobility ≥8 mm and gingival display (GD) ≥4 mm when smiling. The following measurements were taken: length of upper lip (ULL; base of nose to superior border of lip vermillion); lip vermillion vertical length (LVL); lip mobility; and GD. Patients completed questionnaire to determine their pre-operative satisfaction with their smile. Results: 16 patients (2 males) were recruited. Average age was 32.5±12.6 years (range: 18-54). At rest, average ULL and LVL was 14.4±2.0 mm (12-18) and 8.3±1.9 (5-12), respectively. At smile, the corresponding values were 10.7±2.0 mm (7-14) and 4.1±1.3 mm (2-6). Total lip length (ULL+LVL) was 22.8±1.9 mm (20-27) at rest and 14.8±2.0 mm at smile (11-18). Average lip mobility was 9.4±1.4 mm (8-12), and average GD was 5.2±0.9 mm (4-8). Most patients (88%) were “slightly” or “somewhat satisfied” with their smile but 81% were “not at all satisfied” with the amount of gingiva showing when smiling; all patients reported “too much” or “way too much” gingiva showing when they smile. Most patients reported some level of satisfaction with the amount of tooth showing. 63% of patients were “not at all satisfied” with the size/fullness of their upper lip when they smiled. Conclusions: Among patients presenting with a request for gummy smile treatment, most were not satisfied with the amount of gingiva showing (gingival display) when smiling and all of them rated themselves showing too much gingiva during smile. These patient characteristics help explain the patients’ desire and request for treatment of their gummy smile.
