Superior Whitening Efficacy of a Commercial 2% Hydrogen Peroxide Toothpaste

Objectives: To evaluate the in-vitro whitening efficacy of three commercially available toothpastes: a toothpaste containing 2% hydrogen peroxide, a 2-step system containing a fluoride toothpaste and a peroxide whitening gel, and a regular fluoride toothpaste.

Methods: The products tested were a toothpaste containing 2% hydrogen peroxide, a 2-step system containing a fluoride toothpaste and a peroxide whitening gel (Crest® Pro Health™ HD™) and a regular fluoride toothpaste (Crest® Cavity Protection). Artificially stained bovine teeth were brushed with abrasive toothpaste. Each tray containing four teeth was brushed for two minutes with a 1:1 slurry of toothpaste to artificial saliva, rinsed with water, and L*, a*, and b* values were collected with a spectrophotometer. For the 2-step system, the teeth were brushed for 1 minute with the fluoride toothpaste slurry, followed by 1 minute with the whitening gel slurry, according to the product instructions. The peroxide containing slurries were prepared fresh for each individual treatment. This process was repeated 14 times. A whiteness index, ΔW* (where W*= ((L*-100)2+a*2+b*2)1/2) was reported. Analysis of variance was used to compare the mean ΔW* value for each product with p<0.05 indicating significant differences between products.
Results: After 14 treatments, the ΔW* value for the toothpaste containing 2% hydrogen peroxide was -8.07 + 1.17, the ΔW* value for the 2-step system was -3.73 + 1.10, and the ΔW* value for the regular fluoride toothpaste was -2.26 + 0.80. The ΔW* value for the toothpaste containing 2% hydrogen peroxide was statistically significantly (p<0.05) better than the ΔW* value for the 2-step system and for the regular fluoride toothpaste indicating superior whitening performance.
Conclusions: The commercially available toothpaste containing 2% hydrogen peroxide delivers superior in-vitro whitening performance when compared to the 2-step system and the regular fluoride toothpaste.
