MiR-375 is Candidate Biomarker for Malignant Transformation in Oral Lesions

Objectives: Malignant transformation of oral premalignant lesions is the key process in the progression to Oral Squamous Cell Carcinoma (OSCC). Although many clinical and histologic predictors have been used, the risk of malignant transformation in oral premalignant lesions is still difficult to assess. MicroRNAs are small non-coding RNAs associated with cancer initiation and progression. MiR-21 and miR-375 are candidate oncogenic and tumor suppressor miRNA in OSCC, respectively. Our aim is to evaluate these miRNAs as biomarkers of malignant transformation in oral premalignant lesions.
Methods: Formalin Fixed Paraffin Embedded (FFPE) samples from progressive premalignant lesions (PPL) and paired sequential OSCC tumor at the same site were obtained from the same patients (n= 31). In addition, six non-progressive premalignant lesions (NPPL) samples that did not progress to oral cancer after 5 years were used as controls. Total RNA was isolated and Taqman qRT-PCR was used to measure the expression level of miR-21 and miR-375 compared to RNU44 as internal control.
Results: miR-21 had statistically significant higher expression in OSCC than PPL (2 folds difference, p = 0.0066). On the other hand, no significant difference was observed in miR-21 expression level between PPL and NPPL. MiR-375 level was statistically significant higher in PPL than OSCC (p < 0.0001). The expression of miR-375 decreased after the progression from PPL to OSCC. In addition, miR-375 expression level in PPL was clearly lower than in NPPL (8 folds difference, p = 0.0004). ROC curve analysis revealed that miR-375 was able to differentiate between PPL and NPPL (p < 0.0001).
Conclusions: MiR-375 under expression is associated with higher risk of malignant transformation in oral leukoplakia. Therefore, miR-375 can be considered as a candidate biomarker to detect malignant transformation in oral premalignant lesions.
