Obesity, Malocclusion and Quality of life in Brazilian adolescents

Objective: This study aimed to analyze the relation among obesity, malocclusion and quality of life in Brazilian adolescents.

Method: The sample consisted of 124 adolescents at 12 year-old, divided into two groups Overweight/Obesity (O, N=62) and Eutrofic (E, N=62), both genders, from public schools in Bauru-SP, Brazil. The Body Mass Index (BMI) for-age was used for anthropometric assessment, using WHO criteria. Examination were performed by two trained and calibrated examiners (Kappa >0.79). Dental Aesthetic Index (DAI) and Child-Oral Impacts on Daily Performances index (Child-OIDP) were applied. Data were analyzed by t-Student test, Wilcoxon and Pearson linear correlation (P<0.05).

Result: In group O, 83.87% showed malocclusion, being most prevalent Defined and Very severe or disabling malocclusion (30.65%, each one). The malocclusion prevalence in E was 66.13%, highlighting Severe and Defined Malocclusion (27.42% and 24.19%, respectively). DAI scores were 32.48 (± 8.04) in O and 29.32 (± 6.97) in E, being associated with the BMI-for-age (P=0.021). The perceived oral problems most prevalent in O were sensitive tooth (69.35%), position of teeth (58.06%) and tooth color (56.45%), while in E they were sensitive tooth (69.35%), tooth color (62.90%) and position of teeth (61.29%). There were associations between the BMI-for-age and the Overall Child-OIDP, and the performances Eating, Speaking, Sleeping, Studying and Social contact (P<0.05). Correlation was observed between DAI scores and Speaking in both groups (O P=0.009 and E P=0.019). In O there was correlation between DAI and Smiling and Overall Child-OIDP (P=0.029 and P=0.033), while in group E it was observed between DAI and Sleeping(P=0.034).

Conclusion: The malocclusion showed a higher prevalence and severity on overweight/obese adolescents, as well as this condition impacted directly on the daily performances of this group. Therefore it is required a multidisciplinary approach, aiming to improve strategies for prevention and treatment of the evaluated conditions.
