Lip, Oral Cavity and Pharyngeal Cancer Burden in European Union Using Global Burden of Disease Study 2019.

Objectives: To estimate the burden of Lip and Oral Cavity cancer, Nasopharyngeal cancer and Other Pharyngeal cancer (LOCP) among 15-49 year olds, 50-69 year olds and 70+ year olds, in in 1990 and 2019 European Union countries ( EU), using summary statistics and categorising results according to UN Statistics Division Classification.

To compare LOCP DALYs attributable to tobacco use in Ireland and EU between 1990 and 2019.
Methods: In this epidemiological study, EU LOCP data were obtained from GBD 2019. LOCP was classified as B.1.1 Lip and Oral Cavity B.1.2 Nasopharyngeal cancer B.1.3 Other Pharyngeal cancer. LOCP data was analysed according to age and gender. Summary statistics including incidence, mortality rate and DALYS, YLL, YLD were used. LOCP DALYs attributable to tobacco were also calculated and the Irish LOCP DALYs were compared to those of the EU.
Results: Cancer in the category Lip and Oral Cavity (LO) is the most dominant with the incidence decreasing from 6.2 new cases per 100,000 (95% UI: 6.1-6.4) in 1990 to 5.3 new cases per 100,000 (95% UI: 4.6-6.1). Cancers however in the categories Nasopharyngeal (NP) and Other Pharyngeal (OP) are increasing in Europe. NP increased from 1 new case per 100,000 (95% UI: 0.9-1.0) in 1990 to 1.1 new cases per 100,000 (95% UI: 1 -1.3) in 2019 while OP increased from 2.2 new cases per 100,000 (95% UI: 2.2-2.3) in 1990 to 3.3 new cases per 100,000 (95% UI: 2.9-3.8) in 2019. In Europe, Hungary has the highest burden of LOCP with 262.3 DALYs per 100,000 in 2019. When LO attributable to tobacco were analysed, Irish DALYSs decreased by 51.7% between 1990 and 2019. This was in comparison to a 36% decrease in EU countries. However, the percentage decrease of OP DALYs attributable to tobacco in Ireland is above EU average, as Ireland saw a 6.4% decrease compared to 8.3% EU reduction. Finally, it was noted that Years of Life Lost (YLL) is much higher than Years of Life Lived with Disability (YLD), but that YLL has decreased for all three categories since 1990.
Conclusions: There has been a significant increase in the incidence in categories Other Pharyngeal and Nasopharyngeal cancer since 1990. The public must be informed of the association between modifiable risk factors and LOCP. The high mortality rate due to LOCP is worrying as LOCP is largely preventable.
