Increased Genetic Instability in Oral Mucosa of Epidermolysis Bullosa Patients
Objectives: To analyze nuclear aspects of cytotoxicity and mutagenicity in buccal, labial, and lingual mucosa smears from patients with Epidermolysis Bullosa (EB). Methods: Sociodemographic and clinical data were collected through physical examination and a standardized questionnaire. Cells from the buccal, labial, and lingual mucosa were obtained via exfoliative cytology and assessed for the frequency of micronuclei (MN), karyorrhexis (KR), karyolysis (KL), and pyknosis (PN). The Mann-Whitney test was used for statistical analysis. Results: Twenty patients with EB were selected. The control group (n=20) was matched by sex and age with the EB sample. The distribution was similar between men (40%) and women (60%), and most participants were over 19 years old (65%). Quantitative analysis demonstrated higher frequencies of KR and KL in EB patients, particularly in the buccal (p=0.0002 and p<0.0001) and labial (p=0.0005 and p=0.0240) mucosa. Furthermore, in the buccal mucosa, the EB group presented more cells with MN than the control (p=0.0083). Finally, normal cells from all sites were more frequently detected in the control group (p<0.0001, p=0.0008 and p=0.0013). Conclusions: Nuclear aspects of cytotoxicity and mutagenicity were more frequent in EB mucosa. These findings suggest a genetic instability associated with the disease, so we consider exfoliative cytology, which is a minimally invasive technique yielding high-quality cellular samples, a valuable tool for monitoring patients with EB and screening early malignant alterations.
