11-Year Clinical Behavior of Nano-Hybrid-Composite Denture-Teeth

Objectives: To eveluate clinical behavior of nano-hybrid-composite (NHC) denture-teeth (DT) after 11years
Methods: A recall was performed in 17patients with 33removable dentures (partial/complete-dentures, hybrid/implant-supported-overdentures) using 375 DT (SR Phonares II, Ivoclar,Schaan,FL). 4 denture-base materials were used (10xIvoBase-High-Impact, 10xIvoBase-Hybrid, 6xSR-Ivocap-High-Impact, 7xProBase-Cold, Ivoclar). After average observation of 11years of clinical function all DT were examined (swissethics:BASEC-ID2022-D0080) regarding aesthetic, functional and biological properties (table1), using modified FDI-criteria.
Results: After 11years, NHC-DT showed good to very good results regarding surface-luster, surface-staining, plaque-accumulation and color-match (table2). Due to occlusal adjustments and 11years of function 55.7% of DT showed occlusal areas with silk-mat luster (good). 47.2% of DT showed localized plaque-accumulation (good) at predilection-areas (e.g. interdental), but only 5patients generated 66% of localized plaque-accumulation. Regarding material-fracture 94.2% of DT showed very good results. 22 DT had polishable minor(<2mm2) cracks/chippings (2.9%,good) or polishable moderate (>2mm2, no bulkfracture) cracks/chippings (2.9%,sufficient), but 19(86%) of these incidents occurred in 4patients. Chemical bonding of DT to denture-base was excellent (marginal-adaptation=95.2%,excellent/very good). Only at 8 DT (2.1%,good) small isolated gaps/marginal staining at DT to denture-base interface were recorded. Grade-3-score (sufficient) was recorded only in one patient (10 DT, 2.7%), leading to assumption that application errors during denture fabrication were perhaps the cause. Regarding qualitative evaluation of occlusal wear, 80.6% of DT showed good to very good performance. Only 73 posterior DT (19.4%) showed moderate flattening of cusps/occlusal contact surfaces after 11years of function (mastication), which was not noted by any patient. Existing good masticatory function despite abrasion is reflected in high level of patient satisfaction (100%,excellent/very good). Evaluation of mucosa facing DT showed a very good clinical result in 100%.
Conclusions: In summary, NHC-DT (SR Phonares II) are perfectly indicated to be used for long-term restoration of patients with removable partial and complete dentures.
