Clinical Outcome of Posterior Cantilever Resin-Bonded Fixed Dental Protheses (RBFDPs)
Objectives: Cantilever resin-bonded fixed dental prostheses (RBFDPs) present a common treatment option to replace missing teeth in the anterior region, but are rarely used in the posterior region. The aim of the present pilot clinical study was to investigate the clinical outcome of posterior cantilever RBFDPs made from monolithic zirconia ceramic. Methods: Thirty posterior cantilever RBFDPs made from monolithic 3Y-TZP zirconia ceramic (Katana Zirconia ML/HT ceramic, Kuraray Noritake, J-Osaka) were used to replace premolars or first molars in 24 patients. If the first molar needed to be replaced, two cantilever RBFDPs were fabricated for replacement, each of which was bonded to the adjacent mesial and distal abutment tooth. After etching the enamel with phosphoric acid, the RBFDPs were bonded under rubber dam isolation using two randomly selected phosphate monomer containing luting systems (Panavia 21 TC or Tooth Primer/Panavia V5, both Kuraray Noritake). Follow-up investigations were performed after 6 and 12 months after insertion and then annually. Results: Between December 2018 and November 2021, 30 cantilever RBFDPs were inserted. RBFDPs replaced 9 maxillary and 19 mandibular premolars and 1 mandibular molar (replaced with 2 RBFDPs). The mean follow-up period was 31.2 ± 12.6 months. One patient with 1 RBFDP passed away after the 6 months recall. No RBFDPs lost retention or fractured. Therefore, all RBFDPs survived and no other complications such as caries or periodontal problems occurred. Hence, the recorded cumulative survival and success rates were 100%. Conclusions: The clinical outcome of monolithic zirconia cantilever RBFDPs for the replacement of posterior missing single teeth is promising.
