Depressive Symptoms in Partial and Complete Edentulous Patients
Objectives: To investigate the presence of depression symptoms in patients with partial and complete edentulism and determine their associations with dental and denture status. Methods: This cross-sectional study was conducted in 81 patients with partial or complete edentulism attending the Prosthodontic clinic, Faculty of Dentistry, Chulalongkorn University between June to December 2022. Number of natural teeth (NT) was categorized into 0, 1-19, and ≥20, and denture status was classified as wearing or not wearing removable denture. Primary outcome was self-reported depressive symptoms assessed using the 9-item Patient Health Questionnaire (PHQ-9), and the participants were asked to rate each item based on the frequency of symptom over the last 2 weeks using a 0-3 scale (0=not at all, 1=several days, 2=more than half the days, 3=nearly every day). The scores of each depressive symptom were dichotomized into absence (score=0) or presence (score>0) of each symptom. Associations between oral and denture status and each depressive symptom were analyzed using Chi-square or Fischer Exact test and multivariable logistic regression analysis at p < 0.05. Results: The most prevalent depressive symptom was sleeping problem (40.7%), followed by feeling tired (22.2%) and little pleasure in doing things (19.8%). Bivariate analysis showed a significant association between the NT and feeling tired as well as poor eating/over appetite. Not wearing denture tended to be associated with sleeping problem (p=0.087). After adjusting for potential covariates, having at least 20 NT showed lower odds of sleeping problem and feeling tired compared with those with lower than 20 NT. Conclusions: A majority of patients with tooth loss reported at least one depressive symptom. Tooth loss was associated with depression symptoms, particularly sleeping and eating problems. Meanwhile, the association between denture wearing and depressive symptoms needs further investigation.
