Anxiety to Dental Treatment Before and After Non-Surgical Periodontal Treatment
Objectives: The objective of this study was to determine dental anxiety before and after non-surgical periodontal treatment (NSPT). Methods: Patients with periodontitis, of the Clinic of Periodontics of the University of Costa Rica, that had never received periodontal care, were invited to participate in the study. Informed consent was obtained. A questionnaire was applied before and after NSPT. The data collected included: sociodemographic factors, smoking habits, assessment of actual pain and maximal pain in the last month, Modified Corah Dental Anxiety Scale (MDAS), and questions about phobic stimuli. From the electronic health record, the following information was obtained: periodontal diagnosis, Plaque Index (PI), and Gingival Bleeding Index (GBI). Results: Fifty-one patients completed the questionnaires, 19 men and 32 women, age range 46 ±11.16 years. Most of the participants reported having completed high-school (37.3%), followed by university studies (35.3%), primary school (23.5%) and completed other studies (3.9%). Regarding smoking, 46% of the participants did not report this habit. Of the patients who didn’t smoke, 7.8% were former smokers. Patient’s maximal pain in the last month decreased after the completion of NSPT (p=0.002). The most prevalent periodontal diagnosis in the studied population was Generalized Periodontitis Stage III Grade C (27.5%), followed by Generalized Periodontitis Stage III Grade B (23.5%). The average PI before NSPT was 61.4%± 17.6 and the average GBI before NSTP was 39.5%± 23.5. Patients reported that fear to rotatory instruments and ultrasonic scalers decreased after NSPT(p=0.039). Dental anxiety improved after NSPT for total MDAS score (p<0.001) and for all MDAS sub scores (all Ps <0.010). Conclusions: Self-perception of dental anxiety decreased after NSPT and should be regarded as an integral element in routine diagnostic work-up process of periodontal diseases. Its necessary that dentists identify manage patients suffering these conditions, to ensure the success of the periodontal treatment.
