Association of Signs of Periodontitis and Diabetes Mellitus II Complications

Objectives: The aim of the present study was to evaluate the association between self-reported signs of periodontitis (SP) and the presence of diabetes mellitus (DM) complications and their socioeconomic status in a Colombian population.
Methods: A sample of 181 adults diagnosed with type 2 diabetes mellitus participated in this cross-sectional study. Demographics, laboratory data and presence of common diabetic complications were obtained from patient’s medical records. Signs of periodontitis were registered through a questionnaire surveyed to patients. Positive answer to three sets of questions were statistically tested (Bayesian Logit regression model) to select the most reliable group of questions. Associations were statistically analyzed (chi-square test and Fisher exact test, p<0.05).
Results: The sample mean age was 64.1 years, 58.6% were women. And 55.3% stratum two (stratum is the lowest). Metabolic control of patients was inadequate reflected by mean glycated hemoglobin (HbA1c) of 8,0% (SD1.7). Positive for SP were present in 25.4% of the population. The most frequently presented complication was nephropathy (61.9%), followed by neuropathy (35.4%), peripheral vascular disease (29.8%) and cardiovascular disease (CVD) (22.7%); however, in patients positive for signs of periodontitis cardiovascular disease was more common (34.8%). According to the questionnaire most patients use a stick or brush for interproximal cleaning (84.4%) and some have lost teeth due to mobility (27.1%). On the contrary, few have attended a periodontist (2.8%), or have received conventional periodontal treatment such as scaling and root planning (4.4%). Upon relating the SP with complications of DM, a statistically significant association was found with cardiovascular complications (p=0.039).
Conclusions: This study suggests a relation between reported signs of periodontitis and cardiovascular disease as a complication in diabetic patients of a Colombian population.
