Measuring Third Party Disability in Children’s Oral Health – a Review

Objectives: To describe the concept of third party disability in children’s oral health; and to summarize the measurement properties of disease-specific instruments that measure this concept, such as the family component of the Early Childhood Oral Health Impact Scale (ECOHIS-F) and the Family Impact Scale (FIS).
Methods: We performed a targeted and iterative search of the academic literature for studies that describe the development and/or measurement properties of instruments that measure third party disability in children’s oral health. We assessed the sensibility of identified instruments using Bombardier's Framework on Sensibility. This framework covers: the relevance of each instrument to a measurements' purpose, population, and setting, content validity, face validity and feasibility of use. The COnsensus based Standards for the selection of health Measurement INstruments (COSMIN) Study Design checklist guided our appraisal of reliability, validity and responsiveness.
Results: Third party disability is the combined effect of a health condition of a significant other on the functioning, disability and health of a family member who does not have the condition. The ECOHIS-F includes items related to time off work, financial impact, and parental emotions, whereas the FIS and shortened version of the FIS (FIS-8) include items that cover various aspects related to third party disability, such as parental emotions, parental activity, and family conflict. We identified eight studies that assessed the measurement properties of these instruments. The ECOHIS-F, FIS, and FIS-8 show favorable evidence of internal consistency reliability and cross-sectional construct validity. There was some evidence of the test-retest reliability and responsiveness of the FIS and ECOHIS-F, which are important properties for use in longitudinal studies.
Conclusions: The FIS and ECOHIS-F cover different domains of third party disability in children’s oral health. Researchers interested in measuring third party disability, should review the content of these and other instruments before carrying out comparisons or implementing these tools in research.
