Heat Distribution During 980nm-Diode Laser-Assisted SRP in Vitro
Objectives: Diode lasers have been used in non-surgical periodontal therapy for root surface decontamination in conjunction with scaling and root planing (SRP). This study aims to demonstrate the heat transfer (thermography) toward the soft tissues during laser-assisted SRP. Methods: Two fresh slaughtered cow jaws were prepared to maintain soft and hard tissues. The posterior periodontal tissues (n=20) were irradiated for 30 seconds with a diode laser (980nm, Spectralase 980, Spectrum Lasers, San Francisco, USA) utilizing a 320m glass, non-initiated, fiber tip (2W) in a continuous (c.w.) and a pulsed mode (15 Hz, 50% duty cycle). During laser irradiation from the distal to the mesial direction at the buccal tooth surface, temperature changes (in oC) and heat distribution at the soft tissues were captured with a thermographic camera placed at a standardized distance and perpendicular to the beam direction. Thermal heat transfers within the entire irradiation period (in lateral and vertical directions) were measured. Descriptive statistical analysis was performed to present mean values and standard deviations and comparative statistics were calculated using the paired t-test at the 5% statistical significance level. Results: The mean heat temperature changes were 27.47±3.51 (for c.w.) and 28.94±6.28oC (for pulsed mode). The width/height (in mm) for the continuous mode was 8.59±1.94/6.39±1.27 and for the pulsed mode 7.70±1.50/5.44±0.96. Comparisons between c.w. and pulsed modes in the lateral (width) heat transfer and in penetration depth (height) showed statistically significant differences (p=0.089, p=0.006, respectively). Conclusions: Within the limitations of this study, 980nm-diode laser irradiation in the periodontal tissues is distributed toward the soft tissues with potentially uncontrolled heat distribution and temperature increase over the critical threshold for bone and soft tissue necrosis.
