Thermographic Effects in Periapical Tissues During 980nm-Diode Laser Irradiation

Objectives: Diode Lasers can be used for root canal disinfection. This study aimed to demonstrate the heat transfer at the periapical areas of teeth during laser irradiation with a diode laser.
Methods: Mandibular bovine anterior sextants were harvested from freshly slaughtered cows with complete periodontal tissues. Endodontic access was completed on the left central incisor and instrumented up to #70/.04 ProFileÒ rotary file (Dentsply). A diode laser (980nm, 120m, non-initiated tip, 2W) with a continuous (c.w.) and a pulsed mode was utilized to irradiate the root canal in 30- and 60-second intervals (n=20). Maximum temperature and heat transfer were calculated using a thermographic camera placed perpendicular to the irradiation beam. The thermal heat transfer within the entire irradiation period (in lateral and vertical directions) was measured and calculated (in mm). Statistical analysis was provided with a paired t-test between the different laser modes in the different irradiation periods.
Results: The average maximum change in temperature was over 10 degree C. The width/height (in mm) for pulsed mode in 30sec were 5.28±1.20/5.60±1.29 and for 60sec were 4.63±0.79/4.65±0.96, respectively. Similarly, the heat transfer in the c.w.-mode was 5.80±1.37/4.89±1.37 and 5.02±0.56/5.50±0.69, respectively. The statistical comparison test showed no statistically significant differences between pulsed and c.w. modes in 30- and 60-sec irradiation periods.
Conclusions: Within the limitations of this study diode laser irradiation in the periapical areas must be critically considered due to the lateral heat transfer and the temperature increase over the critical threshold.
